The following forms must be completed before you can begin participating in a school sport:
Concussion Protocol Form - Must be signed by player and parent
TSSAA Physical Examination Form - Must be dated after April 15, 2023 and signed by a physician
Sudden Cardiac Arrest Form - Must be signed by player and parent
