The following forms must be completed before you participate in a school sport:

• TSSAA Physical Examination Form - Must be dated after April 15, 2024 and signed by a physician
• Concussion Protocol Form - Must be signed by player and parent
• Sudden Cardiac Arrest Form - Must be signed by player and parent
